Below are a number of forms and policies that are important to complete, or read, if you or your child are a member of Fauldhouse Penguins S.C.

As a parent or guardian of a Fauldhouse Penguins swimmer, we need you to complete and submit all the forms, in the Forms section, below. In some cases your child may also need to sign the form.

Forms are accessed by clicking the hyperlinks. Some hyperlinks will take you through to an on-line form, others will allow you to download a copy of the form for printing, signing and returning electronically via e-mail.

In order to keep better records, to create a better audit trail and to do a wee bit to help the environment, our aim is to become increasingly a paper-less organisation. We’d greatly appreciate your help with this ambition. In order to help you help us we have provided some links to free mobile scanner apps, at the bottom of this page. These apps allow you to scan your signed paper documents, using your smart-phone’s camera,  and in turn will allow you to return them electronically via e-mail.
